After scheduling your initial appointment, please return to this page to download, complete, and return the forms in the files "registration_and_new_ patient_ forms_221129.doc" and "telemedicine_consent_form.pdf" below.
You may send the completed forms to me by encrypted email, fax (844 837 1205)
or regular mail.
