After scheduling your initial appointment, please return to this website (www.dangardnermd.com) to download, complete, and return the forms in the file, "New Patient Forms.doc" (See below).

You may return the completed forms to me by encrypted email, fax, or regular mail. Or you may bring them to your first office appointment.
